The reliability of a determination of the total [51Cr]EDTA plasma clearance (E) (and with it the glomerular filtration rate), by a simplified single injection method (injected dose: 4.5 muCi per kg b.w.) using capillary blood samples (0.2 ml), was investigated in twenty children. From a large database of 51Cr-ethylenediaminetetraacetic acid (51Cr-EDTA) renal clearance determinations using the two blood sample method, 598 patients, aged 1 week to 90 years, were selected. Total plasma clearance of 51Cr‐EDTA, Cl, is widely used as a measure of GFR. Commonly, only the final part of the plasma concentration curve is measured, and a one‐pool clearance (slope‐intercept clearance), Cl1, is computed. The calculation of 51Cr-EDTA clearance by method I (ClI) was based on a transformation of the biexponential time-activity curve into a monoexponential with identical area below the time-activity curve.
Relative to the pharmacokinetic model, 24h urine data provided unbiased GFR predictions. The slope-intercept method for isotopic GFR produced positively biased estimates (MPE 15.5 mL/min/1.73m 2 [95% CI 8.9, 22.2]). In 74 adult patients suffering from various renal diseases, the total [51Cr]EDTA plasma clearance (Cl) was compared to the [51Cr]EDTA clearance obtained on assuming a one-pool system (Cl1). The difference between the two methods was independent of the level of renal function. Similar results were found for one-sample GFR. No clinically relevant differences were found between the plasma clearance of 99m Tc-DTPA and that of 51 Cr-EDTA.
